by the South Eastern Health & Social Care Trust, helps promote
a speedy recovery and a return to normal living after a stay
in hospital, encouraging independence and enabling the user
to remain in the comfort of their own home.

This is a free service provided
for up to six weeks following hospital discharge and tailored
to meet the user's changing needs as their condition improves.
Lisburn Aftercare Service is
intended to supplement other services, not replace them.
Referrals ...
Referrals are taken from hospital
social workers who carry out an assessment of the user's needs.
The hospital social worker will contact Lisburn Aftercare Service
and confirm details and availability with the project co-ordinator.
Who is eligible ...
Anyone:
- Aged 65 and over
- Living alone or
- Living with family members
/ relatives who are unable to help
How It Works ...
A Lisburn Aftercare Service
care worker will be assigned to the user on discharge from hospital.
The care worker's main role is promote a return to independence.
The user's progress will be
assessed by the Lisburn Aftercare co-ordinator at the commencement
of service. A follow up visit will also take place during the
six week placement, with the care worker reporting to the co-ordinator
on a regular basis.
